Steps to sign up for a matching gift program:

  1. Request a matching gift form from your company's human resources department or CSR team.

  2. Complete the form: Fill out the form using your company's official name.

  3. Include a receipt: If you have a receipt for your donation, include a copy with the form.

  4. Submit the form: Send the form to your company's human resources department or CSR team.

What to include on the form? 

  • The donor portion of the form.

  • A copy of your gift receipt.

  • Any other required materials.

  • To designate the matching funds for Kindred Box, include them on the form.
